How does my dad co-sign on my credit card application?

I’m 19 and in college but don’t have a credit card yet, but when I applied they turned me town, so I guess I need a parent to co-sign on it. But it doesn’t say anything about that on the application? What am I supposed to do about that? http://bit.ly/lfT1BA
